| As a courtesy to our patients, we have several methods you may use to pay for your family’s dental services. To avoid misunderstandings, please familiarize yourself with these options: |
| A. Insurance We will accept your dental insurance, file your claim for you and you may pay your portion as the services are rendered. We must be able to verify your coverage in advance. Insurance companies can only estimate the total amount of your coverage, and any balances due remain your responsibility. |
| B. Pay In Advance If you wish to pay for your dental services in full and in advance, we will give you a 10% discount on services. You can of course, file your insurance claim directly. |
| C. Extended Payments We have an arrangement with an outside financing company for extended payments, up to 36 months on your dental services. To qualify for this program, you must have a steady source of income and one major credit card. If you wish to use this method, please ask us for a credit application prior to starting your dental program. |
